What is CellTrak?
• CellTrak enables home health aids and nurses to enter patient specific care data into an intuitive CellTraker™ interface. The data collected is sent back to CellTrak’s secure data servers from the point of care, thereby creating a complete electronic visit record that matches the care plan.
• CellTrak’s back-end servers push patient specific care plans and information to field staffs’ CellTraker™ upon arrival at a patient’s home
• GPS monitoring provides accurate tracking of the exact location of homecare staff and recording the actual mileage for each visit.
• Programmed business rules ensure that all visit records match patient care plans and the visit records have been completed before the patient visit can be completed.
• CellTrak’s intuitively easy-to-use web-based administrative system is easily integrated with the software systems residing at the home healthcare agency - thereby completing the end-to-end automation chain.
